Patients should arrive 15 minutes prior to their schedule appointment time and bring their insurance card, all the current medications, photo ID and a form of payment.

Appointments can be made Monday-Friday during regular office hours, 8:00 a.m. to 5:00 p.m. When patients call, provide a brief description of the medical problem to allow us to schedule accordingly. There is a charge for missed appointments not canceled 24 hours in advance.

Medication refills are given only during the office visits. Therefore it is very important to bring all your medication bottles so that we can give enough refills until the next office visit.

Certain medications, such as controlled substances, cannot be filled over the phone.

Please have insurance cards available each time you visit the office, please note that the copayment and deductible have to be paid prior to the examination.

Patient medical records are confidential. We will not release any medical information unless we have written authorization from the patient. Please make sure that all personal information is accurate and up-to-date.

If you have a medical emergency call 911 or proceed to the emergency Department Immediately. Call us if your need is less urgent and we will do our best to give you an appointment the same day for urgent conditions and within 72 hours, for routine conditions.

We use an online patient portal to allow access to view health records and payments, to request appointments and to retrieve test results.

We provide coordination of care with local psychiatric practices for better and efficient management of major psychiatric illnesses.
